I think that the end should be like a new wilderness, and have some new chunks generated and put into there replacing all the current terrain. I also think it should be smaller, about 2000x2000 blocks. Since it would have all the same rules as the wilderness at -5000y, it wouldn't take too much more coding, and it would also let us have the new biomes generated only in 1.6, so that would be a solution to horses.
Of course, there would have to be a way to get out that lets horses out too (I'm not sure if the normal exit portal in vanilla lets mobs through) so maybe something like the random teleporter which would send you to the overworld, in a random place. This would make it time consuming and tedious to get horses out of the wilderness, so only a few people would have horses. If this does happen, then horses that, if possible, horses that have spawned naturally (not bred ones) have lower health, to make it harder to take the horses to their arranged stables at the player's realms, making it even harder to get the horses away.
This could all be applied to another wilderness in the overworld that may be created as well as the current one (apart from the first paragraph) to solve the problems of horses. All of this would make horses very valuable, so there would be a huge rush for the first overworld horse, and it would be a new objective to liven things up, even if only temporary, for the players that say this they're bored with minerealm.
TL;DR
The end should be a huge wilderness with overworld land instead of end land and horses that would be very hard to get horses out of.
